Nearly 12% of Americans between the ages of 14 and 49 are estimated to have type 2 herpes (HSV-2), or genital herpes infection.[1] One of several well-known sexually transmitted infections (STIs), genital herpes is often asymptomatic—meaning it may not cause noticeable symptoms.2

Genital sores are the most common symptom, but discharge—while less common—can still be an important clue. Changes in vaginal or penile discharge may signal herpes or another infection.

Below, we break down how herpes-related discharge may differ, other STIs that can cause discharge, and when to seek care.

Male vs. Female Herpes Discharge

Genital herpes is most commonly spread through sexual activity and is unique among STIs because many people have mild or no symptoms, and the infection remains in the body for life even though symptoms can be managed.[1] The most common symptom is vesicles—small blisters or sores—that can appear on the genitals, urethra, anus, rectum, or mouth. These blisters may rupture into painful sores and eventually scab over. In some cases, discharge may also occur.

Feature Vaginal Discharge and Herpes Penile Discharge and Herpes
What’s considered normal? Mild-smelling, clear or white, and fairly consistent in texture Typically no discharge other than sexual fluids such as ejaculate or pre-seminal fluid
How herpes-related discharge may appear May increase in volume, develop a stronger or unusual odor, or appear to come from the urethra instead of the vagina May appear clear, cloudy, or blood-tinged, especially if lesions rupture and release fluid; may also have a strong or unusual odor
Common accompanying herpes symptoms Sores or vesicles may appear on or inside the vagina, cervix, genitals, urethra, anus, rectum, or mouth Sores or vesicles may appear on the penis, genitals, urethra, anus, rectum, or mouth
Other possible causes of abnormal discharge Trichomoniasis, gonorrhea, chlamydia, bacterial vaginosis, yeast infection Gonorrhea, chlamydia, urethritis, foreskin infection
When to seek care If you notice sudden, persistent, or unusual changes in discharge, odor, or irritation If you notice any discharge outside of normal sexual fluids or other new symptoms

Because discharge can be caused by herpes or several other infections, testing is the best way to understand what’s going on and determine the right next steps.

Other Symptoms Of Genital Herpes

  • Itching around the genitals
  • Pain or soreness
  • Painful urination
  • Flu-like symptoms (fever, headache, swollen lymph nodes)

Some people also experience tingling or shooting pain before an outbreak.

What STIs Have Discharge As A Symptom?

Discharge is a common symptom across multiple STIs. Testing can help determine the underlying cause.

Chlamydia

  • Increased discharge
  • Painful urination
  • Abdominal or pelvic pain
  • Spotting between periods

Gonorrhea

  • Yellow, green, or milky discharge
  • Thicker consistency
  • Possible bleeding or irritation

Trichomoniasis

  • Foul-smelling discharge
  • Yellow, green, or white appearance
  • Pain during urination or sex

If you notice changes in discharge, early testing and treatment can help prevent complications.

How Is Genital Herpes Contracted?

  • Vaginal, anal, or oral sex
  • Kissing
  • Skin-to-skin contact
  • Sharing personal items (rare cases)

Herpes is most contagious during active outbreaks.

Genital Herpes Diagnosis And Treatment

Diagnosis typically involves testing samples from sores or blood tests for antibodies.

Treatment may include:

  • Antiviral medications to reduce symptoms and outbreaks
  • Suppressive therapy to reduce transmission risk
  • Lifestyle adjustments like safer sex practices and communication

If you need support or treatment, you can also explore online care options and connect with a licensed provider.
